JCAHO data reveals that communication failures play a significant role in causing wrong site surgeries. It therefore mandated a protocol for pre-procedure identification, site marking, and verification by "time out." The Joint Commission for the Accreditation of Healthcare Organizations permits the use of Adhesive Site Markers when used in conjunction with pen marking methods. ( JCAHO ) The VeriSite Patient Labeling System facilitates and enhances the JCAHO mandated protocol.

VeriSite Labels assist in the pre-procedure identification when the label is filled out in the pre-op holding area. Together healthcare staff and the patient and/or the patient's family can review all pertinent information about the patient, procedure, surgeon, intended operative site as the label is completed.

Labels also augment the "Sign Your Site" procedure by, including specific, pertinent information, such as the, patient, surgeon and procedure and confirmation of implant, allowing for more information at the point of entry than indelible markers alone.

Initiate the mandated "time out" by having a designated health care professional read aloud the VeriSite Patient Label so that all members of the surgical team agree with the information, facilitating a proactive method of confirming all pertinent identifying information. Such a process of reconciliation, providing more opportunities to confirm, can result in fewer errors.

VeriSite Labels include a complementary like-numbered document label to be affixed to the patient's chart that verifies and documents policy compliance which can be reviewed during certification audits or internal stopgap audits of your facility's commitment to safe surgical practices and patient safety goals. By such auditing and surveying compliance is increased.
