JCAHO data reveals that communication failures play a significant role in causing wrong site surgeries. It therefore mandated a protocol for pre-procedure identification, site marking, and verification by "time out." The Joint Commission for the Accreditation of Healthcare Organizations permits the use of Adhesive Site Markers when used in conjunction with pen marking methods.
